Finding In-Network Medical Providers
Using the Blue Cross Blue Shield of Arizona Website
1.
Go to Blue Cross Blue Shield of Arizona web site
at www.azblue.com
2. A Guest page will appear. Click on “Search
the Provider Directory” link located in the
middle of the page.
3. On the “Health & Dental Provider
Directory” page, under the ID card sample that says
“Acme Company,” click on the Search button.
Do not click on the Search button that
asks you for the 3-character alpha prefix.
4. If you are on the Choice PPO (80/20) or
Choice Plus (90/10) plans, select one of the choices
marked:
-PPO or -Participating
5. If you are on the Basic Choice PPO
(50/50) or Copay Choice plans, select the choice
marked:
-PPO.
6. Click, “Next.” Search options will
follow.
We
strongly encourage you to access the Blue Cross Blue
Shield of AZ website for the provider directory.
The website is updated monthly by BCBSAZ, listing
any new providers or removing those who have
terminated recently. The 2008 BCBSAZ Provider
Directory books are also available in the Benefits
Office.
Using
the HealthSmart PPO Directory (for Out-of-State
Residents Enrolled in HealthSmart Only)
1. Go to
www.healthsmart.net.
2. Click on the “Search for a Provider” button on
the left-hand side.
3. In the “Search” box, click on “Yes” to agree to
the disclaimer.
4. Enter CM002 as the group number. The screen will
refresh.
4. Under Group Number, click on the CM002
link.
5. Click “Ok” on the pop-up button.
6. Click on the search option of your
choice.

directly at 1-800-687-0500 to find a provider.
For HealthSmart PPO members
visiting Arizona: When you are back in Arizona and
need medical care, please use a BCBSAZ PPO
provider. Have that provider submit the claim to
City of Mesa, not HealthSmart. Your BCBSAZ
group number is CM001. Only have out-of-state
providers submit to HealthSmart PPO with group
number CM002.
