Accessing CHIP
To use CHIP, you must be an insured member with
the City of Mesa or authorized provider with access to a personal computer with
Internet service.
The Employee Benefits Office will provide
each insured with a member ID number and
password information. You will need these
items to logon. This is required to ensure
privacy and safety when you view your personal
claims information.
Passwords expire every 60 days. If you need your password reset, please
contact the Employee Benefits Office by phone at 480-644-3329 during regular
business hours. We do not send out passwords via electronic mail.
