Health ID Cards

NEW! MEDICAL/PRESCRIPTION CARDS SENT IN EARLY APRIL  Members enrolled in the City of Mesa Medical Plan were sent new ID cards in the mail from Medco the first week of April.  These cards combine your medical and prescription drug benefit information.

Two cards were sent to each member's mailing address. If you need additional cards, you can request them from Medco at 1-800-711-0917.  The cards will have only the insured's name, not each individual dependent's name.

Since this card combines information from both Medco, our prescription drug administrator, and the City of Mesa Medical Plan (Mesa Choice Plan), please refer to this guide on your new card:

Dental Cards - Only new hires and members who made changes to their dental plans during Open Enrollment received new cards for 2008.  If you need a replacement card, please contact Employee Benefits to receive a non-personalized dental card.

Vision Cards -Vision cards were not issued by Spectera.  To view your benefits, please logon at Spectera* to view your member benefits. 

*When logging in as a member on www.spectera.com, your member ID made up of 4 leading zeros followed by your 5-digit employee ID. 

HealthSmart Cards - If you and/or your dependents are living outside of Arizona, you will not receive new HealthSmart cards.  Out-of-state plan members were sent new prescription/medical cards from Medco.

If out-of-state members are visiting Arizona, please remember to use a BCBS of Arizona PPO provider for your best benefit. Members will want to present their combine/prescription/medical cards from Medco to their Arizona healthcare provider .

Replacement HealthSmart cards may be requested from the Benefits Office.
