Contract

• Do not fill in the date listed on the CONTRACT, this will be completed by the City of Mesa at time of execution
• Do sign both your name and title on the lines provided, this signature does not have to be notarized or sealed

Payment and Performance Bonds

Bond Agent Requirements:

• FOUR (4) ORIGINAL PAYMENT AND PERFORMANCE BONDS MUST BE SUBMITTED
• Do not fill in the date listed in the 2^nd paragraph of the PAYMENT or PERFORMANCE BOND, this will be completed by the City of Mesa at time of execution
• Do fill in the date listed in the last paragraph of the PAYMENT or PERFORMANCE BOND, this is the date that your bond agent issues the bonds
• Do complete the agency of record name, address, phone and fax information on the lines provided
• Do have your bond agent sign on the line provided.  Agent must affix their seal on each of the PAYMENT and PERFORMANCE BONDS

Contractor Requirements:

• Do sign each of the PAYMENT and PERFORMANCE BONDS
• Do corporate seal each of the PAYMENT AND PERFORMANCE BONDS with your corporate seal. If you do not have a seal, please indicate “No Seal” next to the signature line. IF YOU ARE NOT A CORPORATION, EACH PAYMENT AND PERFORMANCE BOND MUST BE NOTARIZED

Certificates of Insurances

The City of Mesa CERTIFICATES OF INSURANCE must be completed. Accord forms are not accepted.  Completion includes:

• A General Certificate of Insurance (.pdf 1 page)
• A Project Number in the top right hand corner
• Name of Insured
• Name of Insurance Carrier, Policy Number, Policy Period and Actual Coverage amount listed for each of the coverages required. Complete each box separately
• An Owner's Liability Certificate of Insurance (.pdf 1 page) policy for each project (No Exceptions). Also, please note that this coverage is in the City of Mesa’s name not the Contractors
• No modified language on either form

Contracts can be executed with faxed copies of CERTIFICATES OF INSURANCE, however, originals must follow by mail.
